Class, Heinrich

In the previous Chapter it was shown that most accidents are preceded by deviations in the operational process, e.g. Heinrich (Heinrich, 1959), Turner (Turner, 1978), Leplat (Leplat, 1987), Reason (Reason, 1997), etc. Additionally, it was shown that a specific class of deviations is present which is not covered by current pro-active safety indicators. These deviations are characterised by a high likelihood and low perceived safety related consequences and were defined as precursors and re-occur in the operational process of the organization prior to an accident. In order to find these deviations in a real life operation and to eventually find their underlying causes, the concepts of re-occurring deviation and operational process have to be explained in more detail. The various definitions and concepts derived in this Chapter are necessary to understand the next Chapters, which shows how they are applied in practice.

For accident mechanism theory, many domestic and foreign scholars and experts have done much research and put forward a lot of typical accident theory. Such as Domino accident model theory proposed by Heinrich in 1936, Energy transfer accident theory proposed by Gibson in 1961, then derived by Haddon in 1966, Gold mine accident model proposed by Lawrence in 1974, and so on. In domestic, Chen Baozhi put forward the two class of hazard point in 1995, Zhang Li put forward Human error accident model in Complex man-machine system in 1996, He Xueqiu put forward...

Heinrich states that only 10% of accidents are caused by xmsafe mechanical and physical conditions, whereas 88% of accidents are caused by unsafe acts of persons. (The other 2% are classed as unpreventable, or acts of God ). 

We recognise the classical man-environment taxonomy according to Heinrich in Table 6.8. It consists of two classes, i.e. unsafe acts by person and unsafe physical conditions.
