Cimicifuga

Note that the dried rhizome of Cimicifuga racemosa (British Pharmaceutical Codex, 1934 black cohosh) has been used as a bitter and mild expectorant in the form of a liquid alcoholic extract (1 in 1 dose 0.3-2 mL) and is sold as alternative remedy for the treatment of menopausal syndrome at dose of 40-80 mg/day. The active constituents of black cohosh, and, therefore, the precise molecular mechanism of action involved in the climacteric property of Cimicifuga racemosa, are still unknown. The most recent data suggest that the plant is not estrogenic sensu stricto (126).

In 1993, Kusano et al. isolated 2-hydroxy-7-methyl-9H-carbazole (243) from the aerial parts of Cimicifuga simplex Wormsk. ex DC. (224). Six years later, Wu et al. reported the isolation of clausine V (244) from the root bark of C. excavata (43) (Scheme 2.60). The UV spectrum (/max 236, 261, and 305 nm) of 2-hydroxy-7-methyl-9H-carbazole (243) indicated a 2-hydroxycarbazole. This assignment was supported by the IR spectrum [Vmax 1611 (aromatic system), 2700-3000 (OH), and 3400 (NH) cm ]. The presence of a hydroxy function was confirmed by transformation into the corresponding 0-acetate and comparison with 2-hydroxy-7-methyl-9H-carbazole.

There are many examples that illustrate the benefits of this type of herbal combination. Huang Lian [Coptidis rhizoma) descends and clears fire from the Stomach, but it may also suppress Stomach-Qi and cause discomfort in the Stomach. If Sheng Ma [Cimicifugae rhizoma) is prescribed at the same time, it can ascend and disperse the Stomach-Qi constraint and Stomach-fire, thereby reducing the side effects of Huang Lian. 

Xi Jiao (Rhinoceri cornu] can be replaced by Shui Niu Jiao (Bubali cornu]. It can also be replaced by Da Qing Ye (Isatidis folium] and Sheng Ma (Cimicifugae rhizoma] at a ratio of 10 1.
