Treatment principles chronic disease

Alcoholics Anonymous (AA) is a self-help organization for people whose common goal is recovery from alcoholism, and it is the most widely accessed resource for individuals with alcohol problems (McCrady and Miller 1993). The philosophy is based on the concept of alcoholism as a chronic disease that cannot be cured, but one that can be halted by means of complete abstinence. AA has described 12 principles or steps to guide those in recovery. Twelve-step facilitation, a manual-based psychotherapy to promote AA participation (Nowinski et al. 1992), was equally efficacious, compared with cognitive-behavioral and motivational enhancement therapies, in a large study of treatments for alcohol dependence (Project Match Research Group, 1997). [Pg.349]

A broad and vigorons T cell response generally accompanies elimination of HBV as well as HCV infection. By contrast, patients with chronic hepatitis B or C tend to have late, transient, or narrow T cell responses. In a long-term follow-up of HBV-infected patients receiving HPC transplants from HBV-immune individuals, 20 of 31 recipients cleared their HBV infection (Hui et al. 2005). In principle, these results encourage the development of adoptive T cell transfer strategies for the treatment of chronic viral hepatitis. However, it is still controversial whether induction of an efficient T cell response is the cause or the consequence of viral clearance. Furthermore, T cell responses do not only contribute to virus control but also to disease pathology (Rehermann and Nascimbeni 2005). [Pg.284]

Our perspective is thus one of general significance to chronic infection, as well as one that will provide specific pathways to novel treatments of human schistosomiasis, and lymphatic filariasis. These two diseases represent a massive public health problem with 300 million people infected in the world today. Intervention by ablating parasite-specific Tregs in these patients will solve the specific problems of schistosomiasis and filarial diseases, while at the same time proving a principle which will be applicable to chronic infections in general. [Pg.120]

Hyperlipidaemia rarely occurs in cirrhotic patients. Patients with decompensated disease should not be given medical treatment for hyperlipidaemia medications should be withheld during the period of decompensation and then reinitiated according to the principles outlined in Case 3 above, when the patient s synthetic fimction recovers. Note that chronic alcohol use can induce CYP450 enzymes drugs which are metabolised by this system may be cleared more quickly. [Pg.253]
