The mechanism of action of carbamazepine appears to be similar to that of phenytoin. Like phenytoin, carbamazepine shows activity against maximal electroshock seizures. Carbamazepine, like phenytoin, blocks sodium channels at therapeutic concentrations and inhibits high-frequency repetitive firing in neurons in culture (Figure 24-4). It also acts presynaptically to decrease synaptic transmission. These effects probably account for the anticonvulsant action of carbamazepine. Binding studies show that carbamazepine interacts with adenosine receptors, but the functional significance of this observation is not known. 

Carbamazepine induces CYP3A, and the metabolism of risperidone, which mainly involves CYP2D6, may also involve CYP3A. Carbamazepine can therefore reduce risperidone plasma concentrations (SEDA-22, 71). However, since carbamazepine alters the biotransformation of many agents, non-specific enzyme induction has been suggested for the risperidone and carbamazepine interaction (240).

Concurrent administration of any drug metabolized by CYP2C9 or CYP2C10 can increase the plasma concentration of phenytoin by decreasing its rate of metabolism. Conversely, drugs that induce hepatic CYPs can increase phenytoin catabolism. Thus, carbamazepine causes a decrease in phenytoin concentration and phenytoin reduces the concentration of carbamazepine. Interaction between phenytoin and phenobarbital is variable.

Information is limited, but the interactions of remaeemide with carbamazepine, phenobarbital and phenytoin appear to be established, but so far only the carbamazepine interaction seems to have been shown to be of clinical importance. Even so, until more experience has been gained, monitor the effeets of eoneurrent use with phenytoin or phenobarbital. No interaction occurs between remaeemide and valproate or lamotrigine.

There is little evidence to suggest that carbamazepine interacts with digoxin, and topiramate causes only a small reduction in digoxin serum levels. Levetiracetam and tiagabine do not appear to interact with digoxin. 

Although this appears to be the only study, the effects of concurrent use are consistent with both the way carbamazepine interacts with many other CYP3A4 substrates and the way simvastatin interacts with other CYP3A4...
