Paraneoplastic autoimmune syndromes. Tamoxifen is generally considered to be the antiestrogen of choice in both premenopausal and postmenopausal women with metastatic breast cancer who have tumors that are hormone receptorpositive. Tamoxifen is usually administered in 20 mg once-daily doses. There is no advantage for higher doses of tamoxifen. Moreover, long-term administration of very high doses of tamoxifen (e.g., 12 months of 60 to 100 mg/m twice daily) is associated with decreased visual acuity and retinopathy. [Pg.2354]
