Although elderly patients taking the oral antidiabetic drugs are particularly susceptible to hypoglycemic reactions these reactions may be difficult to detect in the elderly. Insulin secretion usually peaks approximately 1 h after a meal, falling off to base levels once again within the following 2 h. 

The signs and symptoms of early sepsis are quite variable and include fever, chills, and a change in mental status with lethargy and malaise. Hypothermia may occur instead of fever. Tachypnea and tachycardia are also evident. White blood cell count is usually elevated, as may be blood sugar. The patient may be hypoxic. Signs and symptoms of early and late sepsis are found in Table 45-2.

Adverse reactions occurring in 3% or more of patients include drowsiness, dizziness, fatigue, tiredness, asthenia, blurred vision, headache, nervousness, confusion, dry mouth, nausea, constipation, dyspepsia, unpleasant taste, purpura, bone marrow depression, leukopenia, eosinophilia, thrombocytopenia, elevation and lowering of blood sugar levels, and weight gain or loss. 

ATP sensitive K channels. They lower down the blood sugar level in type 11 diabetics and non-diabetic individuals. They also decrease the elevated plasma free fatty acid levels. They also sensitize the target tissues to action of insulin by increasing the number of insulin receptors. 

Symptoms of excess include hot flashes, liver damage, elevated blood sugar and uric acid.

In addition, numerous life-threatening adverse reactions have come to the forefront with the newer atypicals, such as hypertension cardiovascular disease, including stroke in the elderly obesity elevated serum cholesterol elevated blood sugar diabetes and pancreatitis. Finally, there is compelling new evidence linking neuroleptic use to premature death.
