Aurothioglucose

Gold compounds can be effective for active RA and may delay or prevent erosive progression of joints in some patients. Gold sodium thiomalate (24), and aurothioglucose (23) are available as injectable preparations. An oral preparation is also available, but is less effective and frequently causes diarrhea. Injectable gold is adrninistered in 10-mg amounts as a test dose, followed by 25 mg once weekly for two weeks, then 50 mg weekly for 20 weeks. 

Gold compounds were first used in 1929 by French doctors to treat rheumatoid arthritis. Two of the most commonly used are aurothiomalate (myocrisin) and aurothioglucose (solganol) (Figure 4.52), given by injection of their solutions in doses of up to 25 mg a week for some years.

GOLD COMPOUNDS. Aurothioglucose and gold sodium thiomalate are given intramuscularly, preferably in the upper outer quadrant of the gluteus muscle. The nurse gives auranofin orally.

Aurothioglucose Solganal Intramuscular 10 mg the 1st wk, 25 mg the 2nd and 3rd wk, then 25-50 mg each wk until total dose is 1 g. Maintenance doses of 25-50 mg every 2-4 wk can follow. Effects occur somewhat sooner than oral gold, but still has long delay (4 mo].
