When administered during the second or third trimester of pregnancy, ACE-inhibitors can cause fetal abnormalities, including oligohydramnios, pulmonary hypoplasia, fetal growth retardation, renal dysgenesis neonatal anuria and neonatal death. 

Cocaine combined with indometacin in a 23-year-old pregnant woman at 34 weeks gestation may have caused fetal anuria and neonatal gastric hemorrhage (390). 

Among 25 infants born after 27 pregnancies in tacrohmus-treated hver transplant patients, there were two neonatal deaths whose conception had been close to the time of transplantation, and very high tacrolimus cord levels were found in one child who presented with neonatal anuria (99). 

Based upon the available evidence it is advised not to use ACEI during pregnancy. In the case of an ACEI-exposed fetus, the pediatrician should be notified of the potential for neonatal hypotension and anuria.

Although it is always difficult to prove that a certain drug or drug class induces fetal toxicify, fhe reports on ACEI-induced fetal renal damage are substantial. The most commonly reported fetal side effect of ACEI is second to thbd trimester onset of ohgohydram-nios and growth restriction, followed by delivery of an infant whose neonatal course is complicated by prolonged and often profound hypotension and anuria...
