A trial of low dose amitryptiline (10-25 mg at night) is worthwhile in patients who do not respond to antispasmodics, and associated depression will be helped by conventional doses of this or other antidepressants. Relaxation therapy, hypnotherapy and cognitive behaviour therapy have a place in selected cases.

The combination of herbal sedatives such as valerian (V, officinalis), passion flower (Passiflora species), and anticholinergic Solanaceae (Atropa belladonna, Datura stromonium, Hyocyamus niger, and Mandragora offici-narum) with alcohol or antihistamines can potentiate the effects of antidepressants, antihistaminics, and antispasmodics, causing drowsiness and obtunding the ability to use machinery (D Arcy, 1993 De Smet et al., 1996). 

The antidepressant drugs were derived via the process of molecular modification from the sympathomimetic agents, antihistamines, antispasmodics (anticholinergics), tranquilizers, and tuberculostatic agents. 

In the phenothiazine area, it has been amply demonstrated that structural changes, in addition to producing expected quantitative changes in tranquilizing activity, also produce unexpected qualitative changes in biological activity. As a result antipruritic, antispasmodic, anticonvulsant, antibacterial, antiemetic, antimotion sickness, anthelmintic, and antidepressant compounds have been developed by molecular modification of the phenothiazines. 

The side effects of these drugs are well known and have been summarized earlier (SED VIII, p. 313). Drugs with selective activity may be classified as antidepressants, antipsychotics, antihistamines, antispasmod-ics or antiparkinsonism agents and yet have anticholinergic actions as side effects. When two or more are given together the unwanted effects are more likely to arise.
