Two a-adrenergic agonists are available on the market. Apraclonidine 0.5% is indicated when a patient on maximum tolerated medical therapy requires short-term additional lOP lowering. Before the introduction of brimonidine, apraclonidine was the only available a-adrenergic agonist and was FDA approved for short-term use. Approximately one-third of patients using apraclonidine showed little or no treatment effect. Brimonidine and the less lipid-soluble and less receptor-selective apraclonidine are 2-adrenergic agonists structurally similar to cloni-dine.
