Abdominal pain levodopa

Adverse effects of the COMT inhibitors relate in part to increased levodopa exposure and include dyskinesias, nausea, and confusion. It is often necessary to lower the daily dose of levodopa by about 30% in the first 48 hours to avoid or reverse such complications. Other adverse effects include diarrhea, abdominal pain, orthostatic hypotension, sleep disturbances, and an orange discoloration of the urine. A 73-year-old woman with a 4-year history of Parkinson s disease developed hallucinations and delusions that were interpreted as secondary effects of levodopa (187). She was given clozapine 25 mg/day and continued to take levodopa. Four days later she complained of abdominal pain. She had raised activities of serum amylase 806 IU/1 (reference range <220 IU/1), lipase 2598 IU/1 (<190 IU/1), and creatine kinase 464 IU/1 (<190 IU/1), and normal concentrations of total and direct bilirubin. Other causes of pancreatitis were ruled out. 

Gastrointestinal Some flowering plants contain levodopa, which is converted to dopamine in the intestinal mucosa and the liver. Dopamine then acts on dopamine receptors to cause nausea, vomiting, arrhythmias, and hypotension. Mucuna gigantea is one such Hawaiian species. A case in which the patient presents with abdominal pain, nausea, and vomiting after the legume ingestion has been reported [67 ].
