Visual lookout

The following clusters of factors were extracted from the data failure with GPS connection GPS not adequately used by user disorientation/lost diverted attention hardware issues no visual lookout . The highest rate of accidents, seven, involved the factor diverted attention due to GPS use. 

In accidents where the injury was classified as fatal, the number of injuries was highest in the events of GPS was not adequately used (N = 6) disorientation/ lost (N = 6) and no visual lookout (N = 6). There was one fatal accident in which diverted attention was an issue. Minor injuries were reported for GPS hardware or software in 10 cases for disorientation or lost in six cases for diverted attention in five cases for visual lookouf there was only one case. 

Operational barriers are the part of the Safety Barrier System (SBS) that involves specific human actions related to the barrier function detection, control, mitigation or emergency shutdovm. Examples of operational barriers could be a manual activation of emergency shutdown systems, firefighting and evacuation. A specific lookout or visual check of an operator that is performed only for safety reasons may be seen as an operational detecting barrier. 

Improper Lookout Delayed recognition due to failure to perform an adequate visual search in a situation that requires a distinct visual surveillance (e.g., in intersections and pulling out of a parking space)...
