Vancomycin interaction with aminoglycosides

Vancomycin Hydrochloride Solutions of vancomycin hydrochloride have an acid pH and incompatibility may reasonably be expected with alkaline preparations or drugs unstable at low pH. Vancomycin interacts with aminoglycosides, loop diuretics, general anesthetics, and neuromuscular blockers. 

Clinically important, potentially hazardous interactions with aminoglycosides, bacitracin, clindamycin, lithium, procainamide, quinidine, tetracycline, vancomycin... 

Drugs that may interact with vancomycin include aminoglycosides, anesthetics, neurotoxic/nephrotoxic agents, and nondepolarizing muscle relaxants. 

Clinically important, potentially hazardous interactions with amikacin, aminoglycosides, galantamine, gentamicin, kanamycin, neomycin, paromomycin, physostigmine, pipecuronium, streptomycin, tobramycin, vancomycin, vecuronium... 

The simultaneous use of two or more antibiotics is recommended in specifically defined situations based on pharmacological rationale. Such combination therapy requires an understanding of the potential for interaction between the antibiotics. For example, vancomycin given alone typically has minimal nephrotoxicity, but it may be nephrotoxic when combined with an aminoglycoside. 

Also avoid giving vancomycin if the patient has taken any aminoglycosides because they increase the potential for ototoxicity (ear) and nephrotoxicity (kidney). If the patient receives vancomycin and aminoglycosides, then you must closely monitor vancomycin serum levels to determine that it remains within the safe range.
