Urate metabolism

At our out-patient stone clinic, 467 consecutive recurrent stone formers, 350 men and 117 women, were examined with regard to urate metabolism. The urate excretion in stone formers was compared with the excretion in 89 healthy subjects recruited from a health survey.

In summary, on the first occasion when the patient was obese and hypertensive, he had a definitely abnormal urate metabolism, but on the second occasion, after wei t loss, when his blood pressure had returned to normal and his urate clearance had improved, his urate metabolism was completely within the normal range. 

Urate metabolism was defined by a technique which has been evolved by many investigators over the years. This involved the determination of urate pool and turnover by isotope dilution following the intravenous injection of labelled urate, with the simultaneous measurement of the percent incorporation of glycine labelled with an alternative isotopic label ( C) and determination of its percent incorporation into urinary, and also into produced urate, within a 7 day period. (Methods detailed in "The Effect of Weight Reduction on Urate Metabolism"). 

It is suggested that the determination of PRPP concentration in erythrocytes is an additional parameter to be considered in understanding the biochemical basis for abnormal urate metabolism demonstrated in some heterozygotes for the moderate HGPRTase deficiency.
