The complex thioamide lolrestat (8) is an inhibitor of aldose reductase. This enzyme catalyzes the reduction of glucose to sorbitol. The enzyme is not very active, but in diabetic individuals where blood glucose levels can. spike to quite high levels in tissues where insulin is not required for glucose uptake (nerve, kidney, retina and lens) sorbitol is formed by the action of aldose reductase and contributes to diabetic complications very prominent among which are eye problems (diabetic retinopathy). Tolrestat is intended for oral administration to prevent this. One of its syntheses proceeds by conversion of 6-methoxy-5-(trifluoroniethyl)naphthalene-l-carboxyl-ic acid (6) to its acid chloride followed by carboxamide formation (7) with methyl N-methyl sarcosinate. Reaction of amide 7 with phosphorous pentasulfide produces the methyl ester thioamide which, on treatment with KOH, hydrolyzes to tolrestat (8) 2[. [Pg.56]

Currently, there is only one antisense drug on the market— Vitravene (active ingredient fomivirsen) for the treatment of cytomegalovirus (CMV)-induced retinitis (inflammation of the retina) in AIDS patients. Fomivirsen has 21 nucleotides complementary to a CMV mRNA sequence, which is necessary for the production of infectious virus. Two examples of experimental antisense drugs are provided in Exhibit 3.15, while Table 3.1 lists other antisense drugs in clinical phase. [Pg.81]

Only solutions of lipophilic antibiotics are able to cross the external barrier of the cornea (drops) and the internal blood-retina barrier (systemic administration) to yield sufficient concentrations in the internal eye (vitreous). Keratitis and ulceration of the cornea can be treated by frequent administration of highly concentrated (fortified) antibiotic drops. In endophtalmitis, emergency vitreous aspirate and in-travitreal and subconjunctival injection of antibiotic solutions with a long half-life is the cornerstone of treatment. These solutions should be prepared by the hospital pharmacy. Empiric topical treatment of minor external eye infections consists of antibiotic containing gels or ointments. [Pg.538]

A somewhat different view is supported by a number of other studies. Behavioural rhythmicity persists in SCN-lesioned rodents under the influence of either periodically available food (Davidson Stephan 1999, Stephan et al 1979) or continuous treatment with methamphetamine (Honma et al 1992). Rhythmicity of at least some circadian genes persists in isolated peripheral tissues for periods of time ranging from several days to two or three weeks (Yamazaki et al 2000, our unpublished results). Removal of the retina (which is known to contain its own circadian oscillator Tosini Menaker 1996) changes the distribution of free-running periods of locomotor activity and the duration of active time (a) (Yamazaki et al 2002a). To pursue our metaphor, these results suggest that circadian signals may be relayed to the periphery by a baton rather than a whip the SCN may indeed be the conductor of an orchestra composed of dozens, if not thousands of potentially independent oscillators. [Pg.111]

The advantage of the partially fluorinated compounds lies more in their potential to mix with silicone oil. Various groups started activities to diversify the portfolio of silicone oils used as long-term endotamponades to enable a reattachment of a detached retina. Dimethylsiloxanes of different viscosities are well established but their use is limited to the treatment of the upper quadrants of the retina. The ocular endotamponades of the future could be a combination of tampo-nading and drug delivery device. Vitrectomy removes the natural vitreous after it has become opaque, inflamed, or unable to keep the retina in place. In many cases, the necessity to remove it is the result of retinal disorders, which are still existent after vitrectomy [50]. In equivalence to the blood-brain barrier, there is also a blood-retina barrier, with the effect that it is difficult to treat retinal disorders systemically. Therefore, the delivery of appropriate drugs via the vitreous cavity would open new treatment options. [Pg.442]

The drug is slowly cleared from vitreous with a half-life of approximately 55 hours in humans and is subsequently cleared from the retina. Measurable concentrations of drug are not detected in the systemic circulation following intravitreal administration. Immediate therapy of CMV retinitis with fomivirsen was more effective in delaying progression than deferred treatment in a recent clinical trial. Concurrent systemic anti-CMV therapy is recommended to protect against extraocular and contralateral retinal CMV disease. Potential side effects include iritis and vitreitis as well as increased intraocular pressure and changes in vision. An interval of at least 2-4 weeks is recommended between cidofovir administration and use of fomivirsen because of the risk of ocular inflammation. [Pg.1129]
