Survival anxiety

Knowledge of the mechanisms whereby the amino acid transmitters produce their effects has been valuable in the development of psychotropic drugs that may improve memory, reduce anxiety, or even counteract the effects of post-stroke hypoxia on brain cell survival. Some of these aspects are considered later.

One of my first narcoleptic patients had been treated by a psychoanalyti-cally inclined psychiatrist who attributed one of her symptoms, cataplexy, to anxiety about sex. Not that she wasn t anxious about sex—in the early 1950s every nice girl was anxious about sex. If you weren t anxious about sex, you weren t a nice girl This logic is a little bit like the European Witch Trials, when those who perished when held under water were considered innocent because they had insufficient witch power to be saved, while those who survived were obviously strongly possessed and needed to be dunked again. If you were not anxious about sex in the 1950s, you were obviously a prostitute. 

Early onset of drug abuse is associated with early sexual activity, crime, and educational failure. Young amphetamine users risk exploitation by adults and are more likely to become involved in criminal or violent behavior and prostitution—having to resort to sex for survival. Consequently, they are also more likely to become infected with HIV or other sexually transmitted diseases and by tuberculosis or other bacterial, fungal, or viral infections. Chronic amphetamine abusers are also more at risk for mental and emotional disorders including anxiety, phobias, and depression. They are at higher risk of suicide. 

Physicians may consciously or unconsciously communicate to the patient whether or not they are concerned about the patient s problem, or are certain about the diagnosis and about the value of prescribed therapeutic measures. In the care of a physician who projects personal warmth, competence, and confidence, the patient in turn feels comfort and less anxiety and optimistically anticipates recovery. The physical condition determines the psychic disposition and vice versa. Consider gravely wounded combatants in war, oblivious to their injuries while fighting to survive, only to experience severe pain in the safety of the field hospital or the patient with a peptic ulcer caused by emotional stress. 

MI usually develops after rupture or erosion of an atherosclerotic plaque within a coronary blood vessel. At this site, the clotting cascade is activated and the resultant thrombus occludes the lumen. In all patients under suspicion of MI, immediate therapy has to be initiated by the emergency physician. To relieve the patient from severe pain and anxiety, morphine and a benzodiazepine need to be given. Antiplatelet drugs and heparin are necessary for preventing further formation of thrombi. Nitroglycerin can be used to reduce cardiac load. When blood pressure and heart rate have stabilized, a p-blocker can be administered to lower cardiac 02 consumption and the risk of arrhythmias. Infusion of lidocaine is required to counter the threat of arrhythmias. The chance of survival of the MI patient depends on the interval between the onset of infarction and the start of therapy.
