Early onset of drug abuse is associated with early sexual activity, crime, and educational failure. Young amphetamine users risk exploitation by adults and are more likely to become involved in criminal or violent behavior and prostitution—having to resort to sex for survival. Consequently, they are also more likely to become infected with HIV or other sexually transmitted diseases and by tuberculosis or other bacterial, fungal, or viral infections. Chronic amphetamine abusers are also more at risk for mental and emotional disorders including anxiety, phobias, and depression. They are at higher risk of suicide. [Pg.41]

Physicians may consciously or unconsciously communicate to the patient whether or not they are concerned about the patient s problem, or are certain about the diagnosis and about the value of prescribed therapeutic measures. In the care of a physician who projects personal warmth, competence, and confidence, the patient in turn feels comfort and less anxiety and optimistically anticipates recovery. MI usually develops after rupture or erosion of an atherosclerotic plaque within a coronary blood vessel. At this site, the clotting cascade is activated and the resultant thrombus occludes the lumen. In all patients under suspicion of MI, immediate therapy has to be initiated by the emergency physician. To relieve the patient from severe pain and anxiety, morphine and a benzodiazepine need to be given. Antiplatelet drugs and heparin are necessary for preventing further formation of thrombi. Nitroglycerin can be used to reduce cardiac load. When blood pressure and heart rate have stabilized, a p-blocker can be administered to lower cardiac 02 consumption and the risk of arrhythmias. Infusion of lidocaine is required to counter the threat of arrhythmias. The chance of survival of the MI patient depends on the interval between the onset of infarction and the start of therapy. [Pg.320]
