After aligning on safety as a strategic objective, developing a patient safety vision, and analyzing the current state, leaders develop a high-level safety improvement plan—or they put in motion a mechanism by which a plan will be developed by a team from within the senior leadership group. The intervention plan identifies areas where the current state diverges materially [Pg.204]

Healthcare organizations often charter an implementation team to do much of the phase II work. Whatever the case, reporting relationships for the people doing this work should be clearly specified, and progress should be monitored, measured, and acknowledged by leadership. [Pg.206]

Taking advantage of such opportunities for engagement fosters the desired culture change. It is important, nonetheless, that leaders provide visible and active support as well as direction, perhaps even an explicit charter, for those who will be doing the phase II design and implementation work. [Pg.207]
