In the late 1970 s and 1980 s, other telemedicine projects funded through the National Institute of Mental Health, the National Library of Medicine, the Memorial University of Newfoundland, and the Australian government took place in Nebraska, Alaska, and Australia. These projects focused on the provision of primary care, specialty care, and psychiatric services to distant or underserved communities. 

Provisions for primary and emergency communications (internal, e.g. with staff, emergency personnel, and external, e.g. with the press, emergency services, hospital, employees relatives, telephone links and radios) require careful assessment. If there is a flammable hazard on site, radios may need to be intrinsically safe or flameproof. 

South Africa. In 1994, the new African National Congress government, fulfilling its mandate to remove the inequities of the apartheid era, introduced free health services for pregnant women and children under five. In 1998, the government extended fee provision to all those using public primary health care services. The removal of fees led to an increase in service utilization, but health workers felt that they were not prepared for the changes, which resulted in unnecessary tensions between workers and patients.
