If a standard form was used in the qualification phase, a system should exist to rank the respondents that are being considered. Such a system can be as simple as the reviewers making a written recommendation summarizing the strong points of their selections. Some companies have developed forms that allow the final scores to be quantified. Appendix C, Sample Toller Assessment—Qjiantitative Format contains such an example. With either method, the client should consider having two or more reviewers discuss the candidates and agree on the recommended finalists. 

About half of individuals who do not respond to pyridoxine will sustain ectopia lentisbj age 5-10 years. Indeed, the diagnosis commonly is made by an ophthalmologist. The median IQ score for B6 responsive and nonresponsive patients is 78 and 56 respectively. Some children present at 1-2 years with psychomotor retardation, convulsions (=20% of cases) and psychiatric difficulties such as depression and personality disorders (=50% of cases).

Brown Obsessive-Compulsive Scale [Y-BOCS] score) and excellent responders (defined as normalization = a Y-BOCS score of <10) in children and adolescents treated with sertraline (March et ah, 1998). The NNTs of 5 and 10, respectively, confirm the implications in the change in mean score in the sertraline group— namely, that an average 6-point Y-BOCS drop (a pre-to post-treatment drop from 24 to 18), which parenthetically is consistent across pediatric and adult OCD SSRl trials (Greist et ah, 1995 Leonard et ah, 1997), leaves most patients in the clinically ill range. 

Treatment response has been traditionally determined by the absence of MDD criteria (e.g., no more than one Diagnostic and Statistical Manual of Mental Disorder [DSM] symptom), or more frequently, by a significant reduction (usually >50%) in symptom severity. Using the latter criterion, patients deemed responders may still have considerable residual symptomatology. Therefore, an absolute final score on the Beck Depres-... 

Severity of Illness. J. M. Roberts (1959] originally reported that higher symptom scores at baseline predicted better ECT response. In contrast, Andrade et al. (1988] and Sackeim et al. (1987a] found no differences between responders and nonresponders in initial severity, while others reported that ECT nonresponders had greater initial severity of depression (Kindler et al. 1991 Pande et al. 1988]. Thus, there is no consensus on how symptom severity, independent of the presence of psychosis or melancholia, is predictive of ECT response. 

In a similar, but larger, double-blind study, 107 patients with unipolar and bipolar depression and 25 patients with PMD were randomly selected to either amitriptyline or imipramine after a 2-week placebo washout [Kocsis et al. 1990]. Doses of both drugs averaged >200 mg/day for 4 weeks. Approximately 67% of the moderately depressed patients with NPMD responded to pharmacotherapy, whereas only 32% of the patients with PMD experienced significant improvement with either drug. However, when severely depressed patients with NPMD [with Hamilton Rating Scale for Depression scores of >27] were compared with the patients with PMD, no difference in response rate was noted.
