Safety precautions errors

Because of the severe toxicities associated with many of the chemotherapy agents, safety precautions must be in place to prevent chemotherapy errors, accidental chemotherapy exposures, and overdosages. [Pg.1277]

D espite many safety precautions within chemical plants, equipment failures or operator errors can cause increases in process pressures beyond safe levels. If pressures rise too high, they may exceed the maximum strength of pipelines and vessels. This can result in rupturing of process equipment, causing major releases of toxic or flammable chemicals. [Pg.353]

Despite all safety precautions, equipment failure, human error and other external events can sometimes lead to increased pressures beyond the safe levels, resulting in a relief event. These possible events are described above, but what are the potential lines of defence and why use relief systems which go beyond the simple use of an SRV The SRV is in fact only a part of the relief system and definitely the most important one. [Pg.31]

The issuance of the batch record for operational use (either in production or research) is usually a step that is not controlled to sufficient levels. Most organizations have the batch records issued by the quality unit (which is the preferred practice), but little attention is placed on the procedure. Some firms have multiple reproductions of the master production record maintained in a file for easy and quick issuance to operations. This practice can have flaws in that extra safety precautions have to be taken when the master production record is updated. Serious errors can occur if a system is not in place to verify that the current revision is utilized. It is highly recommended that copies not be maintained ahead of the time of actual issuance due to the liabilities involved in these critical documents. [Pg.290]

Control deals with errors related to inadequate safety precautions. [Pg.197]

Programmable systems and network technologies have brought a new set of problems to functional safety systems. Software comes with new possibilities for performance failure due to program errors or untested combinations of coded instructions. Hence conventional precautions against defects in electrical hardware will not be sufficient to ensure reliability of a safety system. [Pg.12]
