Alternatively, the current antidepressant may be augmented (potentiated) by the addition of another agent (e.g., lithium, T3), or an atypical antipsychotic (e.g., risperidone). Risperidone has been shown to be effective in combination with fluvoxamine, paroxetine, or citalopram in treatment-resistant depression. Olanzapine and fluoxetine have been found to be safe and effective in treatment-resistant depression.

In most cases, SSRIs are the first choice for drugs to combat OCD. Clomipramine, fluvoxamine, fluoxetine, paroxetine, sertraline, and citalopram are all SSRIs that have been proven effective in reducing OCD symptoms. However, in about 40 to 60% of patients, these drugs do not completely alleviate all the symptoms. When this is the case, a second type of drug called a neuroleptic is often added. Neuroleptic drugs, such as haloperidol, clozapine, risperidone, and chlorpromazine... 

The effect of citalopram on plasma concentrations of risperidone has been prospectively studied in 15 patients with schizophrenia (32). The addition of citalopram did not alter plasma risperidone concentrations.

Citalopram had no effect on plasma concentrations of clozapine (n = 8), risperidone (n = 7), or their active metabolites over 8 weeks (263). However, a possible interaction of clozapine with citalopram has been reported, with increased serum clozapine concentrations, perhaps dose-related (264). 

Dyskinesia occurred for 5 days in an 82-year-old woman after withdrawal of risperidone and citalopram, which she had taken for about 3 months (204). 

Off-label uses of risperidone have been assessed in two studies. In a double-blind study, elderly patients with depression resistant to citalopram, who had previously responded to adjunctive therapy with risperidone, where randomly allocated to continue with adjunctive therapy with risperidone (modal dose 0.8 mg/day n = 32) or to receive placebo ( = 31) [126 ]. Three patients taking risperidone reported headache.
