Protopam - Pralidoxime chloride

PAM Chloride Trade names protopam chloride, or pralidoxime chloride. 2-PAM chloride can be used in the treatment of nerve agent poisoning.

Pralidoxime chloride (protopam chloride), the only AChE reactivator currently available in the U.S., available in a parenteral formulation. The AChE reactivator Hl-6 is available in several European and Near Eastern countries. 

A. Parenteral. Pralidoxime chloride (2-PAM, Protopam), 1 g with 20 mL sterile water. 

PAM CL Pralidoxime, chloride, Protopam , is an antidote to organophosphate poisoning such as might result from exposure to nerve agents or some insecticides. The drug, which helps restore an enzyme called acetylcholinesterase, must be used in conjunction with atropine to be effective. Restores normal control of skeletal muscle contraction (relieves twitching and paralysis). 

Aging is of clinical interest in the treatment of poisoning because cholinesterase reactivators such as pralidoxime (2-PAM, Protopam) chloride are ineffective after aging has occurred. Measurement of metabolites of methyl parathion, p ra-nitrophenol, and dimethylphosphate in the urine has been used to monitor exposure to workers. ... 

Administration of 2-PAM chloride (protopam and pralidoxime) is generally not recommended in carbamate poisoning since it has been shown to interfere with the efficacy of atropine. It was reported that the condition of patients suffering carbaryl-related poisoning deteriorated rapidly following the administration of 2-PAM. Seizure control with diazepam, phenobarbital, or phenytoin may be required. Cardiovascular support and intensive supportive care may be required in serious cases.
