Protamine sulfate, a heparin antagonist, is used in heparin overdose (see also Table 17). 

Because of hemorrhage risk, check hematocrit and test for blood in stool. Administer with caution to menstruating women, or patients with subacute bacterial endocarditis, severe hypotension, liver disease, or blood dyscrasias. Protamine sulfate inactivates heparin and can be used as an antagonist if severe bleeding occurs. 

Hemorrhagic complications of heparin therapy are managed, in part, with the specific antagonist protamine sulfate (36). This agent is discussed in greater detail in the Coagulants section of this chapter.
