Answer At the usual doses it is generally well tolerated though patients may complain of gastrointestinal problems, such as abdominal cramps, nausea, and epigastric pain, as well as of headache. In patients with glucose 6-phosphate dehydrogenase deficiency, primaquine is contraindicated because it can induce hemolysis... 

Primaquine phosphate USP. Tablets marketed in the U.S. contain 15 mg primaquine base per tablet (this may vary elsewhere) doses below are expressed as primaquine base.
