In the remainder of this chapter I shall focus on the relationship between personality and aggressive driving, and illustrate how the various other factors that appear in the model above...

Norepinephrine-Blocking Medications. There is some evidence that exaggerated norepinephrine activity plays a part in the affective instability, aggression, and impulsivity seen in a variety of psychiatric illnesses including the personality disorders. This may in fact explain why the TCAs, which tend to increase norepinephrine activity, have not proved helpful for BPD. Based on this theory, beta blockers, which interfere with norepinephrine activity at the beta-adrenergic receptor, may help in the treatment of impulsivity and aggression in those with BPD. 

Benzodiazepines. Benzodiazepines have not been well studied in patients with Clnster B personality disorders however, they do appear to be used on a relatively common basis. There are two scenarios in which benzodiazepines have been used to treat BPD. First, they have been used on an as-needed basis in the inpatient setting to manage severe agitation and aggression. Lorazepam (Ativan) is often preferred because it can be administered either orally or by injection. Usually, 0.5-2mg of lorazepam can help to cahn an agitated patient with BPD. This can then be repeated as needed every 2-A hours, though care should be taken not to overly sedate the patient.

Delirium is a clinical diagnosis, based on the recent and abrupt appearance of clouded consciousness, with disorientation in time, and then in place and person. The patient can appear perplexed at first, gradually becoming frankly paranoid and aggressive, often with visual hallucinations. In elderly patients without clear localizing signs, acute toxic confusion is most often related to urinary tract infection.
