Recent advances with other anti-inflammatory drugs, ibuprofen and naproxen, which only work by physically blocking the channel to arachidonic acid, mean that the adverse effect of stomach bleeding can be avoided. 

The success of these two drugs provides impetus for the development of other anti-inflammatory drugs aiming at specific inflammatory agents. 

There is no role for other anti-inflammatory drugs such as sodium cromoglycate, nedocromil, keto-tifen, leukotriene antagonists or antihistamines in COPD.

Because of the risk of serious side effects with long-term use, treatment of rheumatic disease with corticosteroids is controversial. Patients with rheumatoid arthritis often take large amounts of aspirin together with other anti-inflammatory drugs which also cause renal papillary necrosis in animals (138 ). In a number of studies, a very high incidence of chronic renal disease and renal papillary necrosis has been observed in these patients. 

In common with other anti-inflammatory drugs aspirin modifies the response to tissue injury and infection. It is often taken for colds and viral infections, but in a controlled study there was no evidence of symptomatic benefit in rhinovirus infections. In addition, antibody-free subjects inoculated with the virus subsequently shed significantly more virus than those treated with placebo. It was suggested that aspirin interferes with the local host defence mechanisms which limit virus replication (179). 

The steady decline in renal function can often be halted and sometimes reversed by correction of fluid and electrolyte disturbances, treatment of hypertension and urinary tract infection, and by the scrupulous avoidance of analgesics (143, 146, 193, 201, 202 ). It is not sufficient merely to persuade the patient to discontinue phena-cetin-containing analgesics. Paracetamol, aspirin and other anti-inflammatory drugs should also be stopped if at all possible. 

Contrary to other elicitors of non-immune anaphylactic reactions (radiocontrast media, neuromuscular blocking agents, non-steroidal anti-inflammatory drugs (NSAIDs)) where there are at least hypothetical concepts regarding the pathomecha-nism of these reactions via increased mediator release (e.g. histamine release, shift in arachidonic acid metabolism from prostaglandins towards leukotrienes, etc.) [26], there is almost no literature regarding the pathomechanism of these reactions after LA application.
