The use of opium dates to 4,000 b.c. At that time it was used for medicinal and recreational purposes mainly via inhalation. Today few opium-containing preparations are used, since the activity of opium is largely attributed to its morphine content. The preparations in use today are those that have constipative effects useful for the treatment of diarrhea. Preparations include pantopon, an injectable hydrochloride of opium alkaloids, and paregoric, a camphorated tincture of opium. Paregoric can be used to treat infants with opioid withdrawal signs following in utero exposure to opioids. 

The more widely used paregoric (camphorated opium tincture) is equally effective and is frequently used in combination with other antidiarrheal agents. Codeine also has been used for short-term symptomatic treatment. 

Opioid derivatives used to treat diarrhea are listed in Table 27-3. Opium tincture (laudanum) and camphorated opium tincture (paregoric) are naturally occurring opiates that are very potent inhibitors of peristalsis. These natural agents are still available for treating diarrhea, but they have essentially been replaced by newer opioids such as diphenoxylate and loperamide. These newer opioids are somewhat less potent but may produce fewer side effects. 

Opium can be taken by mouth or it can be smoked, but since smoking was unknown in Europe and Asia until Columbus brought news of it from the New World, opium smoking did not exist before 1492. Many oral preparations of opium were made in the past. Two that survive into our own times are paregoric, a dilute tincture of opium combined with camphor, and deodorized tincture of opium, formerly known as laudanum, which is more, concentrated.

Papaver somniferum (opium poppy) contains a variety of opioid and related alkaloids, including codeine, morphine, noscapine, papaverine, and thebaine. Crude opium is the air-dried latex obtained by incising the unripe capsules of P. somniferum. Paregoric is ammoniated tincture of opium (Scotch paregoric) or camphorated tincture of opium (English paregoric). The use of these formulations has largely been replaced by use of the purified compounds. 

Also like Da Orta, Acosta noted that various other ingredients were added to bangue for various purposes. Gum areca (Indian betel nut), opium, and sugar were common additives. For those who wished to hallucinate ("enjoy a variety of dreams"), the recipe called for the addition of camphor, clove, nutmeg, and mace. Increased sexual potency could be had by adding amber, musk, and sugar.

Digest the soap and opium in the alcohol three days, then filter and add the camphor and oil, and dissolve. 

Take 4 drachms each powdered opium, gum kino, and ctw hincal 3 drachms each camphor and cloves and 4 drachms aromatic spirits of ammonia. Macerate in 4 pints dilute alcohol. Express and filter. 

Caz horated Tincture of Opium. (Paregoric Elixir). This is a camphorated tincture of opium. Macerate X drachm each powdered opium and benzoic acid, 1 fluid drachm oil of anise, 2 ounces clarified honey, and 2 scruples camphor, in 2 pints dilut alcohol for 7 days, and filter through paper. Cununings Quick Method of Making Paregoric. Tako pulverized opium, 1 drachm camphor gum, 2 scruples benzoic acid, 1 drachm oil of aniseed, 1 fluid drachm clarified honey, 2 ounces hot water and alcohol, 1 pint each. Dissolve the camphor and oil of aniseed in the alcohol triturate the...

Componnd Tincture of Tellitory. Take of bruised pelUtory, 4 drachms camphor, 3 drachms oil of cloves, 2 drachms powdered opium, 1 drachm rectified spirit, 6 fluid ounces digest for 8 days. The product is a most serviceable form of toothache-drops. 

Compound Tincture of Kino. This is made in the same way as other tinctures (see JS O. 35), with tho following ingredients 1 drachm each powdered opium, kmo, and cochineal U draenms each camphor and cloves 1 fluid ounce aromatic spirit of ammonia. aud 1 pint alcohol. 

Chloroform Elixir. Take li ounces each chloroform, tincture of opium, tincture of camphor, and aromatic spirit of ammonia 20 flrops oil of cinnamon, and 2 ounces brandy. This is an excellent mixture for colic. Dose, i fluid drachm. 

Horae Embrocation. Take i ounce each of oil of. spike, oil of monarda (Uorsemint), ami strong ammonia water ounce acetate of opium, 1 ounce chloroform, 2 ounce.s tincture of camphor, 1 ounce oil or origanum, and 2 ounces oil of camphor. This is sfud to bo an excellent preparation.
