Prevention through design operational risk management

Prevention through design concepts must be instituted as an element within an Operational Risk Management System. 

Proposing that Prevention through Design be a specifically defined element in an Operational Risk Management System is also influenced by ongoing transitions observed by this author in the methods to eliminate or reduce the occurrence of human error. 

In ANSI/ASSE Z590.3—2011, the Prevention through Design standard, Addendum G comments on only eight hazard analysis and risk assessment techniques, intentionally. They are Preliminary Hazard Analysis, What-If Analysis, Checklist Analysis, What-If Checklist Analysis, Hazard and Operability Analysis, Failure Mode and Effects Analysis, Fault Tree Analysis, and Management Oversight and Risk Tree (MORT). It was also said in Z590.3 that ... 

The defences-in-depth philosophy presented in Section 7.1 can also be interpreted in terms of Ashby s law of requisite variety. Here, the system s designer has the clear ambition of preventing major accidents through design of barriers. This will make it less demanding for the operations organisation to manage the major accident risks.
