Misoprostol contraindications

The administration of this product by any route is contraindicated in pregnant women because its misoprostol component can cause abortion.

Medicaments Glucocorticoids are contraindicated. Administration of interferon a is more dangerous than helpful. Other medicaments should not be used unless this is absolutely necessary — it must be noted that the disturbed metabolism may alter in unforeseen ways. In contrast, misoprostol (153) and lamivudine (141, 248, 250) showed remarkably positive effects. Especially in fulminant hepatitis B, administration of lamivudine (100 mg/day) proved to be effective. In patients with reactivation due to cytostatic therapy after liver transplantation, lamivudine was much more efficacious than famciclovir. 

Answer D. Sucralfate polymerizes in the GI tract, forming a protective gel-like coating of ulcer beds —healing and 4- ulcer recurrence. Overall, proton pump inhibitors such as omeprazole provide more rapid pain relief and faster healing than antihistamines in ulcers caused by acid secretion. Misoprostol activates PGEj receptors, not omeprazole. Not all GI ulcers are associated with Helicobacter pylori, so a cure rate of 98% is not feasible in principle and (at best) is only in the 95% range in treatment of ulcers established to be associated with the bacterium. Anti-inflammatory steroids cause GI ulcers and would be contraindicated. 

Diarrhea, with or without abdominal pain and cramps, occurs in up to 30% of patients who take misoprostol. Apparently dose-related, it typically begins within the first 2 weeks after therapy is initiated and often resolves spontaneously within a week more severe or protracted cases may necessitate drug discontinuation. Misoprostol can cause clinical exacerbations of inflammatory bowel disease (see Chapter 38) and should be avoided in patients with this disorder. Misoprostol is contraindicated during pregnancy because it can increase uterine contractility.
