Glomerulonephritis membranoproliferative

The patient was a 35-year-old white male with a x-antitrypsin deficiency. He received a combined liver-kidney transplant for cirrhosis complicated by portal hypertension, renal insufficiency secondary to membranoproliferative glomerulonephritis, and combined restrictive and obstructive pulmonary disease at age 18 years. 

Renal disease is seen in 17% of infants with oq-antitrypsin deficiency. It causes massive protein loss, hypoalbuminemia, and renal failure. The kidney disease is an immunological disorder occurring only in patients with liver disease, resulting in a membranoproliferative glomerulonephritis with immunoreactive antitrypsin antigen present, as well as complement and immunoglobulins. It should not be confused with the nonspecific spotty asymptomatic glomerulonephritis or hepatorenal syndrome, commonly seen in cirrhosis.

A 40-year-old man who had taken carbamazepine since childhood suffered for over 10 years from a lupus-like illness with hypocomplementemia, pancytopenia, and splenomegaly (67). He later developed cryoglobulinemia with membranoproliferative glomerulonephritis and raised ANA and pANCA titers. A causative role of carbamazepine in the latter syndrome was suggested by the observation that after withdrawal the antibodies fell and cryoglobulinemia resolved.

Membranoproliferative glomerulonephritis (MPGN) is one of the least-common renal morphologic entities that occurs in older children and adults. Although whites are more frequently affected.
