Mefloquine dosage

The latter adverse effects are more common with the higher dosages required for treatment. These effects may be lessened by administering the drug in two doses separated by 6-8 hours. The incidence of neuropsychiatric symptoms appears to be about ten times more common than with chemoprophylactic dosing, with widely varying frequencies of up to about 50% being reported. Serious neuropsychiatric toxicities (depression, confusion, acute psychosis, or seizures) have been reported in less than one in 1000 treatments, but some authorities believe that these toxicities are actually more common. Mefloquine can also alter cardiac conduction, and arrhythmias and bradycardia have been reported. 

Photodegradation in the solid state takes place only at the sample surface. The degradation rate is therefore dependent on factors that will influence the depth of light penetration, i.e., change the absorption and reflection at the surface (e.g., particle size, crystal modification, color, thickness of powder bed, and coating of the individual particles or the dosage form). Mefloquine, chloroquine, carbamazepine, and furosemide are examples of drug substances that show different decomposition rates dependent on their polymorphous modiflcation.P ... 

In studies of combining ritonavir with fluconazole (2) and ritonavir with mefloquine (3) there were no significant effects, and dosage adjustment is not warranted. 

Sinus bradycardia was seen in 18% of patients taking mefloquine (SEDA-12, 693) (14), occurring some 4-7 days after administration the bradycardia was asymptomatic and lasted about 3-4 days. Transient sinus arrhythmia was also reported, without a need for treatment (SEDA-12, 808). Asymptomatic dysrhythmias were also recorded in a dosage comparison trial (SEDA-16, 308). 

Mefloquine hydrochloride (lariam, mephaquine). The adult dose is 750 mg of the salt taken by mouth followed 6-12 hours later by 500 mg. The corresponding pediatric dose for children weighing less than 45 kg is 15 mg of the salt per kg followed 6-12 hours later by 10 mg of the salt per kg. A single-dose study of this potentially very useful new antimalarial compound in volunteers (14 ) reported only transient dizziness and nausea at relatively high dosage. It is as yet, however, too early to make an accurate assessment of the potential of mefloquine for causing adverse effects, particularly as phototoxicity has been the major adverse effect caused by closely related drugs.
