Inhaled insulin AIR insulin given pre-prandially as an inhalation has been studied in 208 patients with type 2 diabetes and compared with 203 patients receiving pre-prandial injections of insuhn [27 ]. The study was stopped early owing to the withdrawal of AIR insulin. The mean age was 56 years, and just over 50% of patients were men. FEVi fell compared with baseline in those who received AIR insulin. This change was observed at 1 month and persisted, but did not decline further. Forced vital capacity (FVC) was also lower. In one study of long-term outcome of 33 nonsmoking FL dating back at least 6 years (Lalan-CETTE et al. 1993), pulmonary function tests revealed an obstructive profile in 13 subjects, restrictive changes in 1, an isolated decrease in lung diffusion capacity in 3, and normal values in 16. Thin-section CT revealed emphysema in 9 of the patients who had airflow obstruction.

A suspected diagnosis of COPD should be based on the patient s symptoms and/or history of exposure to risk factors. Spirometry is required to confirm the diagnosis. The presence of a postbronchodilator FEV,/FVC ratio less than 70% [the ratio of FEV, to forced vital capacity (FVC)] confirms the presence of airflow limitation that is not fully reversible.1,2 Spirometry results can further be used to classify COPD severity (Table 12-1). Full pulmonary function tests (PFTs) with lung volumes and diffusion capacity and arterial blood gases are not necessary to establish the diagnosis or severity of COPD.

A short-term study of guinea pigs exposed to zinc oxide fume 3 hours/day for 6 days at the threshold limit value (TLV) of 5mg/m revealed pulmonary function changes and morphologic evidence of small airway inflammation and edema. Pulmonary flow resistance increased, compliance decreased, and lung volumes and carbon monoxide diffusing capacity decreased. Some of these changes persisted for the 72-hour duration of postexposure follow-up.
