Loss of containment due to mechanical failure or misoperation is a major cause of chemical process accidents. The design or storage systems should be based on minimizing the hkelihood of loss of containment, with the accompanying release of hazardous materials, and on limiting the amount or the release. An effective emergency response program that can reduce the impacts of a release should be available. [Pg.2306]

As seen in Section 4.1, the major types of losses are the conduction and switching losses. Conduction losses are addressed by selecting a better power switch or rectifier with a lower conduction voltage. The synchronous rectifier can be used to reduce the conduction loss of a rectifier, but it can only be used for forward-mode topologies, and excludes the discontinuous boost-mode converters. The synchronous rectifier will improve the efficiency of a power supply about one to six percent depending upon the average operating duty cycle of the supply. A salient feature of the fluidized bed reactor is that it operates at nearly constant temperature and is, therefore, easy to control. Also, there is no opportunity for hot spots (a condition where a small increase in the wall temperature causes the temperature in a certain region of the reactor to increase rapidly, resulting in uncontrollable reactions) to develop as in the case of the fixed bed reactor. However, the fluidized bed is not as flexible as the fixed bed in adding or removing heat. The loss of catalyst due to carryover with the gas stream from the reactor and regenerator may cause problems. In this case, particle attrition reduces their size to such an extent where they are no longer fluidized, but instead flow with the gas stream. If this occurs, cyclone separators placed in the effluent lines from the reactor and the regenerator can recover the fine particles. One major oil and chemical company has collected data on the cost of accidents, the equipment involved and the cause of the failure for more than 50 years. These data are analyzed annually to help decide where to focus efforts to reduce losses and/or to modify design standards to prevent recurrence. This analysis also identifies failures of the PSM and ESH management system. These can be compared with the cost of delivering the systems and adjustments made to expenditures to improve the cost/benefit balance.
