Isoniazid lymphocyte transformation test

Warrington et al. (1978) found that the lymphocyte transformation test was positive after stimulation with isoniazid, isonicotinic acid, and conjugates of these compounds in 95% of patients who developed isoniazid hepatitis. Healthy controls and isoniazid patients who did not develop liver damage showed a negative test. There was no correlation between the degree of lymphocyte transformation and the severity of liver damage. The appearance of liver damage after isoniazid seems to depend on the presence of cellular hypersensitivity to this medicament. 

Reidenberg and Caccese (1975) found a positive lymphocyte transformation test in presumed isoniazid allergy. Mathews et al. (1971) also found positive lymphocyte transformation tests after isoniazid allergy, especially when the reaction appeared within the first 6 weeks after commencement of therapy and when there was eosinophilia. Positive lymphocyte transformation tests were not significantly associated with fever, personal or familial history of atopy, or allergies to other drugs. 

Assem et al. (1969) describe a patient with jaundice and manifestations of generalized allergic reaction. The lymphocyte transformation test provided evidence of allergy to isoniazid. Warrington et al. (1981) have the opinion that the lymphocyte transformation test is of predictive value in the diagnosis of isoniazid-as-sociated hepatitis.
