Incident analysis initial team

Incident analysis should always be conducted as a fact-finding mission, not a fault-finding one. Good, positive communication is the key ingredient to success. It is understood that incidents do not just happen, they are caused. The cause is often not readily apparent, but it is always the crux of the problem. The cause may be a wide variety of events, or a single episode initiated by the affected employee. Effective analysis will result in a positive attitude toward the job, employment, supervisor, company, and safety procedures. The single most effective way to determine an incident cause is through direct communication between the employee involved in the incident and the analysis team. [Pg.183]

The initial team (employee/supervisor) shall determine if a full team is required. This decision is based on the depth, severity, and complexity of the incident. If a full team is not needed, the supervisor and employee will complete the analysis immediately and carry out a corrective action plan. [Pg.184]
