Incident analysis communication improvement

System responses—actions taken to correct the effects and anticipate the aftereffects of an adverse outcome. Following each event, there is a system response that also needs to be analyzed. How did the system for incident response function How did the management act to improve safety Was an exposed worker properly treated Were communities notified appropriately How did the plant return to a normal state How rapidly did it return Finally, how was the system changed in light of the incident This stage of analysis is considered in Chapter 4. 

In a recent meta-analysis of the six randomized trials, patient transfer for primary PCI was associated with a 42% reduction in the composite endpoint (death/reinfarction/stroke), compared with a strategy of on-site thrombolysis (Fig. 4.10) (95). This was driven mainly by a reduction in the incidence of reinfarction (68% reduction) and stroke (56% reduction), but there was also a trend toward improved survival with PCI. Overall, these findings strongly support community wide adoption of a transfer strategy for mechanical reperfusion, as long as patient transfer can be accomplished within time intervals similar to those described in the randomized trials. The challenge now is to overcome logistical obstacles and replicate these impressive results in clinical practice. 

Molecular spectroscopy is now a mature field of study. It is, however, difficult to find references superior to the classic treatise written by Herzberg nearly 50 years ago (1). The origin of vibrational spectra is usually considered in terms of mechanical oscillations associated with mass of the nuclei and interconnecting springs (9). Vibrational spectroscopy considers the frequency, shape, and intensity of internuclear motions due to incident electromagnetic fields. In the harmonic approximation, the vibrational bands are associated with transitions between nearest vibrational states. When higher order transitions, resonance, and coupling between vibrational motions require analysis, quantum mechanical treatment is mandated (1). Improvements and advancements in poljuner spectroscopy are driven by the many problems of interest in the polymer community. 

Risk analysis is a live document and it should be thoroughly reviewed by the operator whenever there is any change in the system because it is the responsibility of the operator to understand all risks and control measures all the time. Also to improve the quality of risk analysis it is important to validate all hazards/major incident events, likelihood, control measures, and consequence, very rigorously. Proper communication, updating, and education/training are parts of risk management, which is an extension of risk analysis. So far helpful discussions have taken place regarding control measure, but what is control measure The next clause provides the answer. 

The analysis of evasive actions is empirical, and is based primarily on historical information firom past incidents. These rates may be pessimistic because increased attention is now being given to emergency response plans for CPI facilities. These response plans should improve the scope and response to evasive actions [e.g., CMA s CAER (Community Awareness and Emergency Responses) program].
