Oleic acid is the common name for (Z)-9-octadece-noic acid and is a nearly colorless liquid at room temperature with a melting point of 5-7°C. Oleic acid is practically insoluble in water but has been used as a solvent for oral delivery of hydrophobic drugs in soft gelatin capsules. Oleic acid is used to solubilize ritonavir in Norvir and Kaletra soft gelatin capsules (see section Microemulsion Oral Formulations ). Testosterone undecanoate is an ester prodrug of testosterone intended for oral administration in hormone replacement therapy, and is solubilized in oleic acid in 40 mg Restandol 40 mg soft gelatin capsules. [Pg.3348]

Although information is limited, the pharmacokinetic interaction between the ethinylestradiol component of combined hormonal contraceptives and nelfinavir or ritonavir appears to be established and is likely to be clinically important. Similar decreases in plasma levels of ethinylestradiol caused by other drugs have resulted in reduced efficacy and reliability of combined oral contraceptives, and one retrospective report suggests that this has occurred with nelfinavir. It seem likely that the reduced contraceptive levels seen with fosamprenavir, lopinavir and tipranavir were due to the concurrent use of ritonavir (as would be common in practice). Similarly, although no interaction was reported with saquinavir, and raised contraceptive steroid levels were reported with amprenavir and atazanavir, in practice these drugs would be given with ritonavir (as a pharmacokinetic enhancer), and so the levels of combined hormonal contraceptives can reasonably be expected to be reduced. Musculoskeletal Changes in bone mineral density and bone turnover have been studied in 50 patients taking lopinavir -f ritonavir with either zidovudine - - lamivudine or nevirapine [78 ]. Bone mineral density rapidly fell in both the femoral neck and lumbar spine after the start of therapy, and there was greater loss after 24 months in those who took zidovudine -I- lamivudine. Osteocalcin and the urine deoxypyridinolinexreatinine ratio increased to the same extent in both groups.
