Do you perceive the major cause of accidents to be unsafe work conditions

For example, I then wrote that since 1980, no articles had been published in the magazine Professional Safety that had performance measures or performance measurement in their titles or abstracts. A further search was made using effective and effectiveness as the key phrases. Two articles were found The June 1981 issue of the magazine contained How Do You Know Your Hazard Control Program Is Effective, written by Fred A. Manuele the February 1989 issue included Using Perception Surveys to Assess Safety System Effectiveness by Charles W. Bailey and Dan Petersen. 

Conduct periodic organizational perception surveys about hazard control efforts.
