In most cases, SSRIs are the first choice for drugs to combat OCD. Clomipramine, fluvoxamine, fluoxetine, paroxetine, sertraline, and citalopram are all SSRIs that have been proven effective in reducing OCD symptoms. However, in about 40 to 60% of patients, these drugs do not completely alleviate all the symptoms. When this is the case, a second type of drug called a neuroleptic is often added. GFJ has been shown to increase the exposure of carbamazepine (175), cisapride (176-179), fluvoxamine (184), losartan (188), methadone (189), scopolamine (191), and sertraline (192). However, only the interaction of GFJ with carbamazepine and cisapride seems to be clinically relevant. No alteration in exposure was observed for clozapine (180,181), heophylline (195), halo-peridol (196), and omeprazole (190). Reports of increased pharmacokinetic parameters of clozapine, theophylline, and haloperidol suggest that an interaction is unlikely to be clinically relevant. Contradicting results were reported for itraconazole (185-187), digoxin (75,183), and sildenafil (193,194). An increased effect on concomitant use of diclofenac and GFJ was observed in rats (182). Overall, the clinical relevance for this drug class appears to be low. 

During the course of a study of the salts formed by saccharin with quinine, haloperidol, mirtazapine, pseudoephedrine, lamivudine, risperidone, sertraline, venlafaxine, zolpidem, and amlodipine, a 1 1 cocrystal of saccharin and piroxicam was detected [68]. In the crystal structure, the asymmetric unit was found to consist of one saccharin molecule and one zwitterionic piroxicam molecule that were linked by two sets of N—H O hydrogen bonds. The piroxicam-saccharin synthons were in turn linked through bridging C—H O hydrogen bonds. Interestingly, the drug substance solubility out of the cocrystal was found to be comparable to that of the marketed piroxicam product. 

Cyt 2D6 metabolizes haloperidol, risperidone, thioridazine, sertindole, olanzapine and clozapine common substrates - fluoxetine, paroxetine, sertraline, venlafaxine, amitriptyline, clomipramine, desipramine, imipramine, nortriptyline, propranolol, metoprolol, timolol, codeine, encainide, flecanide. Common inhibitors - paroxetine, sertraline, fluoxetine.

A 76-year-old man reported difficulty in starting to swallow after taking haloperidol (6 mg/day), sertraline (200 mg/day), lithium, and temazepam for 6 weeks for recurrent depression (206). 

Urinary retention and severe constipation occurred in a patient who started to take sertraline when already taking haloperidol and clonazepam (669). After a week, she discontinued sertraline and her symptoms disappeared in a few days. 

Surprisingly, in a double-blind, randomized, placebo-controlled study, there were no significant differences in plasma haloperidol concentrations when sertraline (n = 18) or placebo (n = 18) were added at 2, 4, 6, and 8 weeks (671).

Benztropine Seven patients developed delirium when given fluoxetine, paroxetine, or sertraline with benztropine in the presence of perphenazine or haloperidol. Other patients remained symptom-free. 

Haloperidol Serum may be increased slightly by sertraline. Fluoxetine, paroxetine and fluvoxamine may inhibit the metabolism of haloperidol and cause extrapyramidal symptoms.
