Failures prevention

The closer one is to the failure, the more its direct effects are apparent. The cumulative effects of failure are often overlooked in the rush to fix the immediate problem. Too often, the cause of failure is ignored or forgotten because of time constraints or indifference. The failure or corrosion is considered just a cost of doing business. Inevitably, such problems become chronic associated costs, tribulations, and delays become ingrained. Problems persist until cost or concern overwhelm corporate inertia. A temporary solution is no longer acceptable the correct solution is to identify and eliminate the failure. Preventative costs are almost always a small fraction of those associated with neglect.

Failures (and Near-Failures) Preventable by Better Design or Construction... 

We will discuss four main topics stuck pipe prevention, drillstring failure prevention, ROP optimization and real-time use of drilling mechanics. 

Drillstring Failure Prevention. Drillstring failures are mainly due to vibrations, shocks and neutral point positioned too close to the drill pipes. They result in drillstring washouts and twist-offs. 

However, in patients with renal failure there is a strange and currently unexplained observation in relation to non-renal clearance. If this is measured for some compounds it also is found to be depressed even though it is the kidney that is diseased and not the liver The picture becomes a little clearer if the same non-renal (presumed hepatic) clearance is measured again in patients after renal dialysis when the hepatic clearance has been found to have risen to control values. Recent animal experiments have demonstrated that the circulating inhibitor of hepatic cytochrome P450 may be parathyroid hormone. Parathyroidectomy of rats with chronic renal failure prevented the reduction in liver cytochrome activity (see Michaud et al., 2006). 

The performance of an instrument should be reviewed on a regular basis to ensure that the instrument is reliable and continues to comply with the requirements specified in the user requirements. The review should determine whether the instrument is maintained in a validated state. The review should include records for use, maintenance, services, and performance verification testing. In case the records indicate that the instrument is more prone to certain types of failure, preventive maintenance may be desirable to avoid system failure during operation. The review can also provide useful information to prioritize instrument replacement.

Maintenance tasks are either implemented proactively in order to prevent or minimize the chance of failure or reactively to correct a situation following failure. Preventive maintenance plans are derived from the EMEA process that will define the tasks and task frequencies required in order to maintain system reliability, consistency, and capability. Maintenance programs both prevent failures and prolong the useful life of equipment. In the case of safety—critical equipment and components, failure prevention is of primary concern, however prolonging useful life will also reduce failure frequency and thus contribute to overall reliability.

The use of event-chain models of causation has important implications for the way engineers design for safety. If an accident is caused by a chain of events, then the most obvious preventive measure is to break the chain before the loss occurs. Because the most common events considered in these models are component failures, preventive measures tend to be focused on preventing failure events— increasing component integrity or introducing redundancy to reduce the likelihood of the event occurring. If corrosion can be prevented in the tank rupture accident, for example, then the tank rupture is averted.
