Failure modes and effects analysis worksheet

These same positive trees are very useful feeder documents for many types of hazard analysis. For example, the item layout for failure mode and effects analysis worksheets can be taken directly from the positive tree for the project. Positive trees can easily be converted into negative trees for troubleshooting or accident analysis. [Pg.105]

The analytical technique used for this study was the failure mode and effects analysis (Figure 14-3 shows the FMEA worksheets filled out for the above Sprinkler example). [Pg.163]

The information recorded on the PHA worksheet, together with the PHA report, will greatly facilitate the performance of other benehcial system analyses (such as the subsystem hazard analysis, the failure mode and effect analysis, and the operating and support hazard analysis) that may be accomplished during the remaining phases of the product life cycle. [Pg.84]

The PSM Rule requires that a FMEA be performed by a team, all of whose members participate in the analysis. The most practical means of performing the FMEA is to prepare blank worksheets on viewgraphs or on a large display screen. For each equipment item, the PrHA team reaches a consensus on its failure modes and their causes, effects, detection methods, compensating provisions, severity (if desired), and any remarks or action items. [Pg.66]

FMEA uses special tables and charts to log data during the analysis. One element of a typical worksheet is a component description. The worksheet identifies the individual or combinations of components being analyzed. The worksheet has a column for failure mode. Additional columns list effects on other components and effects on the system. The... [Pg.528]

FMEA is a very methodical tool that looks at every component in the system (or subsystem) under study and then identifies the ways the component can fail (failure modes). Each failure mode is further analyzed to determine what would be the effects of that particular failure mode on the system. Results are posted in tabular form (very similar to the hazard analysis worksheet). [Pg.224]
