The mechanic decides that a bandaid is in order and goes to the lunchroom, where the first-aid kit is kept. While the mechanic is in the process of getting a bandaid, the supervisor shows up and wants to know what is happening. The mechanic tells the supervisor that he has a minor cut and does not offer any detail about how the incident occurred. After a little investigation, the supervisor gets pertinent information about how the incident occurred. With the help of the mechanic, the supervisor determines that the injury is minor. [Pg.183]

This incident received a thorough investigation. As in most accidents, a variety of factors contributed to the situation. For this accident, a variety of improvements were recommended. Some of the main points brought forth during the investigation were as follows  [Pg.183]

I do not necessarily agree with all of these recommendations, but they do point to a lack of safety culture. In a work environment with a developed safety culture, this accident would not have happened. A thorough job safety analysis (JSA) would have been in place that would eliminate the cable injury. If this step broke down, the mechanic would have been wearing gloves. If that step broke down, the relationship between the mechanic and the supervisor would have been one where the mechanic would have received quicker medical attention. [Pg.184]
