Encephalopathy disulfiram

The neurotoxic effects of disulfiram have been compared with those of carbon disulfide, a disulfiram metabolite (9). The results suggested that carbon disulfide may be responsible for the behavioural and neurological adverse effects of disulfiram. If so, other toxic effects of carbon disulfide might follow administration of high doses of disulfiram, such as parkinsonism, psychotic behaviour, and encephalopathy.

Drug overdose An acute peripheral neuropathy with quadriparesis, lancinating pain, sensory loss, paresthesia of the distal limbs, and a vocal fold palsy occurred 1 month after a disulfiram overdose of 130 tablets in a 49-year-old woman [51 ]. A severe toxic encephalopathy with coma, convulsions, and quadriparesis occurred in a 35-year-old man who took an overdose of disulfiram [52 ].

Known side effects involving the CNS during disulfiram treatment comprise acute encephalopathy accompanied by memory disturbances, deficient judgement, inability to concentrate and emotional instability (U). The incidence of encephalopathy has been estimated at 2% (54).
