OATPs are found in the intestine, liver, kidney and brain. They tend to move or pump drugs from regions of high concentration to those of lower concentration. Examples of drugs that are influenced by the activity of OATPs include digoxin, enalapril, hydrocortisone, lovastatin and the anti-HIV protease inhibitor drugs saquinavir and ritonavir. [Pg.858]

A 50-year-old man with heart failure and a valve prosthesis, taking digoxin, furosemide, and spironolactone, was given enalapril 5 mg/day. Two days later, after increasing the dose to 10 mg, he developed a fever with cough and clear sputum, with a normal chest X-ray. Enalapril was withdrawn and 24 hours later the fever resolved. It recurred immediately after rechallenge. [Pg.1212]

An elderly man taking digoxin, enalapril, amitriptyline and temazepam was treated for monomorphie ventrieular taehyeardia, firstly with procainamide, later replaeed by a 2-mg/minute infusion of lidoeaine, to which oral amiodarone 600 mg twiee daily was added. After 12 hours his lidocaine level was 5.4 mg/L (therapeutie levels 1.5 to 5 mg/L), but 53 hours later he developed a seizure and his lidoeaine level was found to have risen to 12.6 mg/L. A tomography brain sean showed no abnormalities that could have caused the seizure and it was therefore attributed to the toxic lidocaine levels. ... [Pg.262]

A 74-year-old patient stabilised on acenocoumarol, enalapril, piretanide, and digoxin had the piretanide and enalapril switched to furosemide and fosinopril. Eleven days later, he presented with dark faeces (melaena) and had a low haemoglobin. Fosinopril and acenocoumarol were stopped, and then enalapril and acenocoumarol were restarted. On gastrointestinal endoscopy, no explanation for the melaena was found, and his haemoglobin level had returned to normal 15 days later. This case was attributed to possible potentiation of the effect of acenocoumarol by fosinopril, but as the drugs were not taken alone, the interaction is not proven. [Pg.361]

Enalapril 20 mg daily for 30 days had no significant effect on the pharmacokinetics of digoxin 250 micrograms daily in 7 patients with congestive heart failure. ... [Pg.904]
