If there was a strong Safety Culture including a robust MOC system, to examine risk most of these incidents would not have happened. In fairness, many of these incidents occurred prior to full implementation of the concept of MOC as mandated by OSHA s PSM Standards. Therefore, each Good Intention was not always checked by another individual for any additional risks, which may have been created.
