Educating the Patient and Family The nurse evaluates the patient s ability to understand the therapeutic drug regimen, ability to care for himself or herself in the home environment, and ability to comply with the prescribed drug therapy. In the community practice setting, pharmacists frequently provide the initial access to the health care system. They are often asked by patients to make sense of the technical language and help them achieve the full benefit from their therapeutic regimen. In the hospital and clinic, pharmacists often counsel patients either during the hospital stay or as part of the discharge planning. Here also, the ability to influence the patient to receive the full potential of the medical therapy is dependent on the pharmacist s ability to understand and communicate with both health care professionals and patients. 

No matter how therapeutic wards are artificial environments, separating people from the real world. The longer this continues, the more likely they ll see themselves as full-time patients , reliant on psychiatric services. By planning for discharge from the day of admission, you ll encourage independence, create safe and supportive discharge plans... and appease stressed-out bed managers.
