Pharmacokinetics The pharmacokinetics following oral administration of a single dose or multiple doses of diclofenac/misoprostol to healthy subjects under fasted conditions are similar to the pharmacokinetics of the 2 individual components. Food decreases the multiple-dose bioavailability profile of both formulations.

However, for other drags, there exists no food effect. A case in point is diclofenac. The administration of a 150 mg diclofenac hydrogel-based capsule dose within 30 min following a standardized breakfast was shown to minimally affect the bioavailability of dicolfenac relative to administration under fasted conditions. 

The patient should be told about the prescribed dose and when to take the tablets. The pharmacist should check whether Mrs PJ suffers from asthma or has a previous history of peptic ulcer or upper gastrointestinal disease. The patient should be advised to take the medication with, or after food as diclofenac may cause stomach irritation. 

With further questioning, it may be possible to clarify the symptoms and to ascertain whether one of the drugs is likely to be causing the problem. You should check that Mrs PJ is taking her diclofenac with food so as to reduce the risk of gastric irritation. If you decide that the problem is likely to be dyspepsia, in most cases an antacid may help. In this case, however, an antacid could not be taken at the same time as either sulfasalazine or diclofenac. This is because both are enteric-coated tablets and the presence of an antacid may lead to the premature dissolution of the coating due to the presence of an alkaline pH. Therefore, the use of an antacid would not be a suitable suggestion.

Diclofenac is best absorbed in the presence of food. Administration in the fasting state results in erratic absorption.
