Clomethiazole liver

Clomethiazole is structurally related to vitamin B 1 (thiamine) and is a hypnotic, sedative and anticonvulsant. It is comparatively free from hangover it can cause nasal irritation and sneezing. Dependence occurs and use should always be brief. When taken orally, it is subject to extensive hepatic first-pass metabolism (which is defective in the elderly and in liver damaged alcoholics who get higher peak plasma concentrations), and the usual t) is 4 h (with more variation in the old than the young) it may also be given i.v. 

When treating alcoholic liver diseases, it should be taken into account that one is generally dealing with chronic alcoholics. Therefore, one should be aware of the fact that a withrawal syndrome might occur, possibly requiring the application of clomethiazole, haloperidol or clonidine. In this complicated phase, the administration of zinc is likewise recommended. (138)... 

Clomethiazole (6)(see Table 5.7) may produce nasal irritation and sneezing on administration, conjunctival irritation, headache, gastrointestinal disturbances, nausea, vomiting, fever, cough, and tachycardia. It is contraindicated in patients with chronic pulmonary insufficiency, renal failure, or liver disease. 

Clomethiazole. Clomethiazole is rap idly absorbed from the gastrointestinal tract and produces peak plasma concentrations in about 15-45 min after oral administration. It is broadly distributed in the body and is extensively metabolized in the liver and excreted in the urine with only very small amounts of unchanged drug appearing in the urine. 

Cimetidine inhibits the liver enzymes concerned with the metabolism of diazepam, alprazolam, chlordiazepoxide, clomethiazole, clorazepate, flu-razepam, nitrazepam, triazolam, and zaleplon. As a result their clearance from the body is reduced and their serum levels rise.
