The term brittle asthma was first used in 1977 to describe patients who maintained a wide variation in PEER despite high-dose inhaled steroids. This has been reviewed and described by Ayres et al. (1998) based on the magnitude of diurnal PEFR variability, rather than the pattern of variability. They also describe two subclasses maintained wide PEF variability (type 1, >40% diurnal variation for >50% of the time over at least 150 days) or sudden onset of acute airway obstruction (type 2, occurring in less than 3 hours without obvious trigger). [Pg.78]
