Digoxin Antineoplastics

Plasma digoxin levels may decrease when the drug is administered with bleomycin. When bleomycin is used witii cisplatin, there is an increased risk of bleomycin toxicity Pulmonary toxicity may occur when bleomycin is administered with other antineoplastic drugs. Plicamycin, mitomycin, mitoxantrone, and dactino-mycin have an additive bone marrow depressant effect when administered with other antineoplastic drugs. In addition, mitomycin, mitoxantrone, and dactinomycin decrease antibody response to live virus vaccines. Dactinomycin potentiates or reactivates skin or gastrointestinal reactions of radiation therapy There is an increased risk of bleeding when plicamycin is administered witii aspirin, warfarin, heparin, and the NSAIDs. 

Glyceryl trinitrate overdose should be treated with the patient s head lowered. Other measures include respiration maintenance, use of plasma expanders, and electrolyte balance. Withdrawal of heparin treatment or dose reduction should be performed with the overdose of heparin. Protamine sulfate may be used to reduce severe bleeding. Heparin should be used with caution with glyceryl trinitrate, aprotinine, alcohol, tobacco, and ACE inhibitors. Nifedipine should be used with care when coadministering with immunosuppressants, magnesium salts, tobacco, digoxin, antineoplastics, calcium channel blockers, antihistamines, antifungals, antiepileptics, antiarrhythmics, and alcohol. 

Simultaneous administration of sucralfates with other nonantacid medication should be given after an interval of at least 2 hours. Sucralfates interact with rantidine, cimetidine, digoxin, quinidine, theophylline, and warfarin. Sulfasalazine should be administered with caution with antibacterials and antineoplastics.179... 

CAUTION. Bioavailability of digoxin gets altered due to the presence of such drugs as antacids, antineoplastic agents, cholestyramine resins, dietary fibre, erythromycin, neomycin, tetracyclines, metoclopramide, sulphasalazine and propantheline. 

Other studies confirm that a 50% reduction in serum digoxin levels (using beta-acetyldigoxin) occurred in patients given cyclophosphamide, vincristine, procarbazine and prednisone (COPP) cyclophosphamide, vincristine and prednisone (COP) cyclophosphamide, vincristine, cy-tarabine and prednisone (COAP) and doxorubicin, bleomycin and prednisone (ABP). These effects disappeared about a week after cytotoxic therapy finished. Radiation has a smaller effect. Digitoxin absorption does not seem to be affected by antineoplastics. ...
